New patient forms help us get to know you and your needs. By downloading and filling them out before your appointment, we can streamline your visit and get a better idea of how we can best serve you.

We’re happy to work with most dental insurance plans and will submit claims directly to your insurance company on your behalf. While Century Dental is generally considered an out-of-network provider, we may participate with select Delta Dental Premier plans.

Our team will help you understand your dental benefits, provide estimates for recommended treatment, and make the most of the coverage available to you. Any portion not covered by your insurance is the patient’s responsibility.

At this time, we are unable to accept Oregon Health Plan (OHP).

Can’t pay for your treatment all at once? No problem! We accept financing options through Cherry, CareCredit, and Lending Club, so you can pay at your own pace and get the treatment you need, when you need it.
