New patient forms help us get to know you and your needs. By downloading and filling them out before your appointment, we can streamline your visit and get a better idea of how we can best serve you.

We accept all insurance plans, though we are not in-network with providers (with possible exception of Delta Premier). Your insurance will be billed directly, and you'll simply pay the difference between our fee and your insurance coverage. Unfortunately, we are unable to provide services to Oregon Health Plan members at this time.

Can’t pay for your treatment all at once? No problem! We accept financing options through CareCredit and Lending Club, so you can pay at your own pace and get the treatment you need, when you need it.
